The IL-17 family has six subtypes: IL-17A, IL-17B, IL-17C, IL-17 D, IL-17E (also known as IL-25) and IL-17F.

Interleukin-17 (IL-17) and IL-17-producing cells have been shown to play important roles in inflammation and the immune response. IL-17 is believed to be mainly produced by T helper 17 (Th17) cells, a unique helper T-cell subset different from Th1 and Th2 cells. Other subsets of T cells such as γδT and natural killer T (NKT) cells have also been found to produce IL-17 in response to innate stimuli. IL-17 acts as a proinflammatory cytokine that can induce the release of certain chemokines, cytokines, matrix metalloproteinases (MMPs) and antimicrobial peptides from mesenchymal and myeloid cells. This leads to the expansion and accumulation of neutrophils in the innate immune system and links innate and adaptive immunity in vivo. Furthermore, increasing evidence indicates that IL-17 and IL-17-producing cells are involved in the pathogenesis of various diseases such as allergies, autoimmune diseases, allograft transplantation and even malignancy. They may also play protective roles in host defense against infectious diseases and promote induction of cytotoxic T lymphocyte (CTL) responses against cancer. Targeting of the IL-17 axis is under investigation for the treatment of inflammatory disorders.

What Do Elevated Levels of Interleukin-17 (IL-17), Serum Indicate?

Interleukin-17 (IL-17) is a family of pro-inflammatory cytokines, with IL-17A and IL-17F being the most well-studied members. These cytokines are primarily produced by Th17 cells, a specialized type of T-helper immune cell. IL-17 plays a crucial role in protecting the body from bacterial and fungal infections, particularly at mucosal surfaces. However, elevated serum levels of IL-17 can also signal chronic inflammation, autoimmunity, or systemic immune dysregulation.


What Does an Elevated IL-17 Level Mean?

Elevated IL-17, especially IL-17A and IL-17F, indicates that your immune system is in a pro-inflammatory state. This is often associated with:

  • Autoimmune disease activity

  • Chronic inflammatory conditions

  • Active infection

  • Immune response to cancer

  • Inflammation linked to metabolic or cardiovascular disease


Conditions Associated with Elevated IL-17 Levels

1. Psoriasis

IL-17A plays a central role in the development of psoriasis. It acts on epithelial (non-immune) cells, particularly keratinocytes in the skin, where it:

  • Increases cell proliferation

  • Alters differentiation

  • Induces antimicrobial peptides and inflammatory chemokines

This cascade leads to the red, scaly plaques characteristic of psoriasis. The effectiveness of IL-17A–blocking biologics (e.g., secukinumab, ixekizumab) highlights the cytokine's importance in the disease.

2. Autoimmune Diseases

Elevated IL-17 levels have been implicated in several autoimmune disorders, including:

  • Rheumatoid arthritis (RA)

  • Systemic lupus erythematosus (SLE)

  • Multiple sclerosis (MS)

  • Inflammatory bowel disease (Crohn’s disease and ulcerative colitis)

  • Primary Sjögren’s syndrome (pSS) — where IL-17F has been linked to elevated autoantibody levels

In these diseases, IL-17 contributes to tissue damage, sustained inflammation, and recruitment of immune cells to affected organs.

3. Infections

IL-17 is essential in fighting certain infections, especially those involving:

  • Fungi, like Candida albicans

  • Extracellular bacteria, such as Staphylococcus aureus

During infection, IL-17 helps recruit neutrophils and promotes the release of antimicrobial peptides. Elevated levels during infection are typically temporary and return to normal once the pathogen is cleared.

4. Cancer and Tumor Immunity

In some types of cancer, IL-17 levels can rise due to immune system engagement with tumor cells. While IL-17 may help fight tumors in some cases, in others, it may contribute to tumor-promoting inflammation.

5. Cardiovascular and Metabolic Disease

Studies have linked IL-17A to acute coronary syndrome and atherosclerosis. Its presence correlates with systemic inflammation, suggesting IL-17 might contribute to vascular damage and plaque formation. IL-17 has also been implicated in obesity-related inflammation and insulin resistance.


Symptoms Associated with High IL-17

IL-17 itself doesn’t cause symptoms directly, but high levels often occur in the context of:

  • Joint pain and swelling (RA)

  • Skin lesions or flares (psoriasis)

  • Gastrointestinal issues (IBD)

  • Fatigue, fever, or systemic inflammation

  • Autoimmune antibody production (e.g., ANA, RF, anti-SSA)


Diagnostic and Clinical Use of IL-17 Testing

While IL-17 is not commonly used as a standalone diagnostic test, elevated serum IL-17 may:

  • Support the diagnosis of an inflammatory or autoimmune disorder

  • Help monitor disease activity or treatment response

  • Be part of research or specialized immune panels in complex cases
