Vitamin B12 (cobalamin) is a cobalt-containing vitamin that catalyzes the synthesis of the amino acid methionine. B12 is also crucial for healthy red blood cells, brain and nervous system function, DNA regulation, and metabolism. B12 can be found in animal products, such as meat and eggs, but it can also be produced by gut bacteria and rapidly absorbed into the intestines.

In fact, gut-derived B12 production contributes up to 31% of the daily recommended intake for this nutrient.

Bacterial vitamin B12 is synthesized by Pseudomonas denitrificans, Bacillus megaterium, Propionibacterium freudenreichii, Bacteroides fragilis, Prevotella copri, Clostridium difficile, Faecalibacterium prausnitzii, Ruminococcus lactaris, Bifidobacterium animalis, B. infantis, B.longum, and Fusobacterium varium. Furthermore, Lactobacillus plantarum and L. coryniformis isolated from fermented food produce vitamin B12, and Bifidobacterium animalis synthesizes vitamin B12 during milk fermentation.

Elevated levels of Vitamin B12 (Cobalamin) on a BiomeFx test report may signify:

  1. Potential health concerns: Unlike other B vitamins, elevated B12 levels can be a sign of serious underlying conditions and warrant further investigation .
  2. Possible underlying conditions:
    • Hematologic disorders such as chronic myelogeneous leukemia, promyelocytic leukemia, polycythemia vera, or hypereosinophilic syndrome
    • Liver diseases including acute hepatitis, cirrhosis, hepatocellular carcinoma, or metastatic liver disease
    • Myeloproliferative disorders 
  3. Causes of elevation:
    • Enhanced production of haptocorrin (a B12 binding protein) in some blood disorders
    • Cobalamin release during hepatic cytolysis
    • Decreased cobalamin clearance by an affected liver 
  4. Need for further testing: An observed elevation of cobalamin in blood merits a full diagnostic workup to assess the presence of disease .
  5. Interpretation considerations:
    • Testing of non-fasting specimens or recent use of B12 supplements can result in elevated plasma vitamin B12 concentrations

Low levels of Vitamin B12 (Cobalamin) on a BiomeFx panel indicate a potential deficiency or impaired absorption of this essential vitamin, which plays a critical role in red blood cell formation, nervous system function, DNA synthesis, and energy metabolism. Since certain gut bacteria contribute to B12 metabolism and absorption, low levels may signal gut dysbiosis, poor dietary intake, or malabsorption issues.

Potential Causes of Low Vitamin B12 Levels

  1. Dietary Deficiency

    • B12 is not naturally produced by the body and is mainly found in animal-based foods (meat, eggs, dairy, fish).
    • Vegetarians and vegans are at higher risk of deficiency.
  2. Poor Absorption Due to Gut Issues

    • Dysbiosis or low levels of B12-producing bacteria (e.g., Lactobacillus and Bifidobacterium).
    • Low stomach acid (Hypochlorhydria): Acid is required to release B12 from food. Conditions like H. pylori infection, chronic antacid use (PPIs), or aging can impair this process.
