Transitional B cells carry high levels of CD38 and IgM, markers that identify them as newly formed immune cells.
These cells play an important role in shaping the B-cell repertoire and ensuring immune tolerance. During maturation, transitional B cells undergo rigorous screening to eliminate autoreactive cells, reducing the risk of autoimmune disease. Because they are sensitive to disruptions in bone marrow function and peripheral immune regulation, transitional B-cell counts provide meaningful insight into immune system balance.
Clinically, elevated levels of transitional CD38+IgM+ B cells may indicate autoimmune activation (such as in systemic lupus erythematosus or rheumatoid arthritis), chronic inflammation, splenomegaly, or immune reconstitution after B-cell–depleting therapies.
