Lambda
Lambda – Immunofixation, Serum (NMH/LFH)
What is Lambda?
Lambda refers to one of two types of light chains (the other being kappa) produced by plasma cells, a type of white blood cell responsible for making antibodies (immunoglobulins). Each antibody contains either kappa or lambda light chains, but not both. Measuring lambda light chains helps assess abnormal antibody production, which can be a sign of certain blood or bone marrow disorders.
Why is Lambda Tested in Serum Immunofixation?
The Immunofixation Electrophoresis (IFE) test identifies abnormal proteins (monoclonal immunoglobulins or M-proteins) in the blood. These are often produced in excess in disorders like:
-
Multiple myeloma
-
Monoclonal gammopathy of undetermined significance (MGUS)
-
Waldenström’s macroglobulinemia
-
Light chain amyloidosis (AL amyloidosis)
The Lambda test specifically detects the presence of monoclonal immunoglobulins with lambda light chains. The panel also checks for immunoglobulin heavy chains (IgG, IgA, IgM) and kappa light chains to determine the exact type of monoclonal protein present.
What Does an Abnormal Lambda Result Mean?
-
Elevated or restricted lambda bands may indicate monoclonal lambda light chain production, often associated with plasma cell or B-cell malignancies.
-
A predominant lambda band without a matching kappa band suggests a monoclonal gammopathy with lambda restriction.
-
In contrast, a polyclonal increase in both kappa and lambda is usually a sign of inflammation or immune activation, not cancer.
Normal vs. Abnormal Pattern
| Pattern | Interpretation |
|---|---|
| Polyclonal (kappa and lambda present in balance) | Normal immune response |
| Monoclonal (only lambda, sharp band) | Possible plasma cell disorder (e.g., multiple myeloma) |
| No detectable bands | Normal or below detection threshold |
Clinical Follow-Up
If a monoclonal lambda light chain is detected, additional tests may be ordered to determine:
-
The quantity of monoclonal protein (via serum protein electrophoresis or free light chain assay)
-
Bone marrow biopsy to evaluate plasma cell population
-
Kidney function, since light chains can damage the kidneys
Summary
The Lambda marker on a Serum Immunofixation (IFE) panel helps identify abnormal antibody production associated with blood cancers and related disorders. When interpreted alongside kappa and heavy chain findings, it provides crucial information about the type and significance of any monoclonal protein detected.
What does it mean if your Lambda result is too high?
Elevated lambda levels on a Serum Immunofixation panel—especially when appearing as a sharp, distinct band—typically indicate the presence of a monoclonal gammopathy, where a single clone of plasma cells is overproducing immunoglobulins with lambda light chains. This is often seen in conditions such as multiple myeloma, MGUS (Monoclonal Gammopathy of Undetermined Significance), Waldenström’s macroglobulinemia, or light chain (AL) amyloidosis. It suggests a possible clonal plasma cell or B-cell disorder and usually requires further testing, such as serum free light chain analysis, bone marrow biopsy, and imaging studies, to determine the underlying cause and disease extent.
