The DHEA-S (Dehydroepiandrosterone Sulfate) to cortisol ratio is a marker used in assessing adrenal gland function and can provide insight into the balance of hormones produced by these glands.
DHEA-S is an androgen precursor produced by the adrenal glands, which is then converted into more potent androgens or estrogens within tissues.
Cortisol, also produced by the adrenal glands, is a steroid hormone pivotal to the stress response, glucose metabolism, and inflammation control.
An optimal balance between DHEA-S and cortisol is crucial for maintaining health, as it reflects the state of adrenal function and the body’s response to stress.
A skewed ratio, with either elevated DHEA-S or cortisol, could indicate various conditions, including adrenal fatigue, Cushing's syndrome, adrenal hyperplasia, or adrenal insufficiency.
Evaluating this ratio helps clinicians understand if there's an overproduction or underproduction of these hormones, guiding diagnostic and therapeutic decisions.
What does it mean if your DHEA-S : Cortisol Ratio result is too high?
A high DHEA-S (Dehydroepiandrosterone Sulfate) to cortisol ratio suggests an imbalance in adrenal hormone output. This could mean that the body is producing a normal or elevated amount of DHEA-S, the precursor to androgens, relative to cortisol, or that cortisol levels are low in comparison to DHEA-S levels.
Several conditions might lead to such an imbalance:
- Adrenal Fatigue or Insufficiency: If the adrenal glands are underactive or fatigued, cortisol production may be insufficient, leading to a relatively higher DHEA-S to cortisol ratio.
- Chronic Stress: In some cases, prolonged stress can exhaust the adrenal glands' ability to produce cortisol, while DHEA-S production remains comparatively stable or less affected.
- Adrenal Hyperplasia or Tumors: Overgrowth of adrenal tissue or tumors may lead to increased DHEA-S production without a corresponding increase in cortisol.
- Cushing's Syndrome: Though typically associated with high cortisol levels, some forms of Cushing's syndrome may manifest as an increased DHEA-S to cortisol ratio due to the overproduction of adrenal androgens.
- Polycystic Ovary Syndrome (PCOS): Women with PCOS may exhibit elevated levels of DHEA-S, which could alter the ratio if cortisol levels are normal or low.
An increase in DHEAs/Cortisol ratio, was found in patients suffering from panic disorders. Suspect: An abnormal physiological response to stress, with shifting of the steroidogenic pathway to DHEA at the expense of cortisol.
Possible treatment options/lifestlye changes:
- Stress reduction: chronic stress can fatigue the adrenals. Rest, exercise, meditation, relaxation exercises
- Dietary changes: Balance blood sugar, lower calorie, high complex carbohydrate and high fiber diet
- Nutritional supplements: High-grade multivitamin and mineral. Additional Vitamin C, Vitamin B5, Vitamin B6, and zinc
- Herbal Support: American or Korean ginseng (Panax spp.), Siberian ginseng (Eleuthrococcus senticosus), Withania (Withania somnifera, aka ashwagandha), Licorice (Glycyrrhiza glabra) to prolong the half-life of cortisol, Sarsaparilla (Smilax spp.) is a cortisol precursor.
It's important to note that while the DHEA-S to cortisol ratio can provide valuable information about adrenal function, it should be interpreted in the context of other clinical findings and laboratory results. A healthcare provider can provide a more comprehensive assessment to determine the cause of the hormonal imbalance and the appropriate treatment.
