Deamidated Gliadin Peptide (DGP) IgA
Deamidated Gliadin Peptide (DGP) IgA is a highly specific antibody used to assess immune reactivity to gluten. When gluten is consumed, it is broken down into smaller components, including gliadin. In individuals with celiac disease, gliadin undergoes a process called deamidation, making it more immunogenic and triggering an immune response. The production of DGP IgA antibodies reflects this abnormal immune activity.
DGP IgA is particularly valuable in detecting early or active celiac disease, and in some cases may become elevated before other markers, such as tissue transglutaminase (tTG) antibodies. It is also useful in identifying individuals with gluten sensitivity who may not yet meet full diagnostic criteria for celiac disease.
What Does an Elevated DGP IgA Mean?
High levels of DGP IgA are strongly associated with:
- Celiac disease
- Gluten-driven intestinal inflammation
- Increased intestinal permeability (“leaky gut”)
- Active exposure to gluten in sensitive individuals
In celiac disease, elevated DGP IgA correlates with immune-mediated damage to the small intestine, which can impair nutrient absorption and lead to deficiencies in iron, vitamin B12, folate, and fat-soluble vitamins.
Symptoms Associated with Elevated Levels
Elevated DGP IgA may be linked to a wide range of symptoms, including:
- Bloating, diarrhea, or constipation
- Abdominal pain
- Fatigue or brain fog
- Unexplained nutrient deficiencies
- Weight loss or difficulty gaining weight
- Skin conditions such as dermatitis herpetiformis
However, some individuals may have minimal or no symptoms, despite ongoing intestinal damage.
Important Testing Considerations
For accurate interpretation, testing should be performed while consuming a gluten-containing diet. Avoiding gluten before testing can lead to falsely low or normal results.
DGP IgA is often evaluated alongside:
- Tissue Transglutaminase (tTG) IgA
- Total IgA levels (to rule out IgA deficiency)
- DGP IgG (especially if IgA deficiency is present)
What to Do If Your Levels Are High
If DGP IgA is elevated:
- Do not eliminate gluten before confirmatory testing unless advised by your healthcare provider
- Follow up with additional testing (such as tTG antibodies or intestinal biopsy if indicated)
- Work with a healthcare provider to confirm diagnosis and develop a plan
If celiac disease is confirmed, treatment requires a strict, lifelong gluten-free diet, which allows the intestine to heal and reduces long-term health risks.
Why This Marker Matters
DGP IgA improves detection of gluten-related disorders, particularly in early disease or when other markers are inconclusive. Identifying gluten-driven immune activation early can help prevent chronic inflammation, nutrient deficiencies, and long-term complications.
Lab Results Explained and Tracked
What does it mean if your Deamidated Gliadin Peptide (DGP) IgA result is too high?
Elevated DGP IgA is a clinically meaningful indicator of gluten-triggered immune activation and is strongly associated with celiac disease and intestinal inflammation.
