White blood cells (WBCs) in cerebrospinal fluid (CSF) are a crucial component of the immune system's defense in the central nervous system. CSF is a clear, colorless fluid that surrounds the brain and spinal cord. The presence of white blood cells in CSF indicates an immune response within the central nervous system and can have various clinical implications. The CSF white blood cell count is a diagnostic measure used to assess the presence and quantity of white blood cells in the CSF.
It's essential to differentiate between different types of white blood cells in CSF, as this can provide valuable diagnostic information. The two main types are lymphocytes and neutrophils, with lymphocytes being more common in chronic infections and autoimmune disorders, while neutrophils are typically associated with acute infections.
CSF white blood cell counts are an essential part of diagnosing and monitoring neurological conditions. They are typically assessed through a CSF analysis, which involves collecting a sample of CSF through a lumbar puncture (spinal tap) and then analyzing the cellular components, including white blood cells.
Understanding the type and quantity of white blood cells in cerebrospinal fluid is crucial for healthcare providers in diagnosing and managing a wide range of neurological conditions.
Normal CSF has very few WBCs (0–5/mm3).
Lab Results Explained and Tracked
What does it mean if your CSF WBC result is too high?
An increase of white blood cells in CSF indicates infection, inflammation, or bleeding into the cerebrospinal fluid.
Some causes include:
→ Abscess (a collection of pus that has built up within the tissue of the body)
→ Encephalitis (inflammation of the brain)
→ Hemorrhage (bleeding from a damaged blood vessel)
→ Meningitis [an inflammation (swelling) of the protective membranes covering the brain and spinal cord. A bacterial or viral infection of the fluid surrounding the brain and spinal cord usually causes the swelling. However, injuries, cancer, certain drugs, and other types of infections also can cause meningitis.]
→ Demyelinating disease such as Multiple sclerosis [Demyelinating disease is any condition that causes damage to the protective covering (myelin sheath) that surrounds nerve fibers in your brain, the nerves leading to the eyes (optic nerves) and spinal cord. When the myelin sheath is damaged, nerve impulses slow or even stop, causing neurological problems.]
→ Tumor/Malignancy [The term "malignancy" refers to the presence of cancerous cells that have the ability to spread to other sites in the body (metastasize) or to invade nearby (locally) and destroy tissues.]
→ Neurosarcoidosis (characterized by inflammation and abnormal cell deposits in any part of the nervous system—the brain, spinal cord, or peripheral nerves. It most commonly occurs in the cranial and facial nerves, the hypothalamus (a specific area of the brain), and the pituitary gland.)
→ Other infections
