Your result shows the amount of copper measured in one random urine sample relative to urine creatinine. Dividing by creatinine helps account for how dilute or concentrated the urine was, but this remains a spot measurement. It is not the same as measuring the total amount of copper excreted during a 24-hour urine collection.

For this test, the desired range is <87 mcg/g creat. A result at or above 87 indicates increased copper excretion in that sample. It does not reveal the cause and does not, by itself, diagnose Wilson disease or prove that copper is accumulating in the body.

Higher values may occur with Wilson disease, cholestatic or obstructive liver disease, acute inflammation, pregnancy, estrogen use, and copper-chelating treatment. Treatment context is especially important: chelating medications are intended to move copper into the urine, so a high result during treatment has a different meaning than a high result in someone who is not receiving treatment.

A value below 87 is within this laboratory’s one-sided reference range. The test does not have a separate low cutoff. A low spot urine value alone cannot diagnose copper deficiency or establish that the body’s copper stores are adequate.

Interpret the result with the reason the test was ordered, symptoms, liver tests, serum copper, ceruloplasmin, pregnancy status, medications and supplements, and any known diagnosis or treatment for Wilson disease. When Wilson disease is being evaluated or monitored, a 24-hour urine copper collection may be used as part of the broader assessment.

Clinical Takeaway

A high random urine copper result means that increased copper excretion was detected in that sample. It does not identify the cause or directly measure how much copper is stored in the body.

A high result does not identify the cause or diagnose Wilson disease by itself. Increased urinary copper can occur with Wilson disease, cholestatic or obstructive liver disease, acute inflammation, pregnancy, estrogen use, and copper-chelating treatment. In someone receiving chelation therapy, increased urinary copper may reflect the intended movement of copper out of the body.

Your healthcare provider may interpret the result alongside serum copper, ceruloplasmin, liver tests, symptoms, medications, supplements, and the reason the test was ordered. An unexpected result may lead to repeat testing or a 24-hour urine copper collection.

What does it mean if your COPPER, RANDOM URINE result is too low?

Your random urine copper result is below 87 mcg/g creat and therefore falls within the desired range for this test.

Because the laboratory provides only an upper cutoff, a value far below 87 is not automatically considered abnormal. However, a low or within-range spot urine result cannot confirm that the body’s copper stores are adequate, diagnose copper deficiency, or completely exclude a disorder of copper metabolism.

If copper deficiency or Wilson disease is being evaluated, this result should be interpreted alongside other findings such as serum copper, ceruloplasmin, liver tests, symptoms, nutrition, medications, supplements, and sometimes a 24-hour urine copper collection.
