The RC-IV Activity / CS Ratio evaluates the performance of Respiratory Chain Complex IV (also known as cytochrome c oxidase) compared to the mitochondrial content of the sample. Complex IV is a crucial enzyme inside mitochondria, responsible for transferring electrons to oxygen—the final step in the electron transport chain and an essential part of ATP (energy) production.
By expressing Complex IV activity relative to Citrate Synthase (CS), this marker shows whether mitochondria are functioning efficiently per unit of mitochondrial mass. This makes it a valuable functional indicator, especially in situations where mitochondrial content may be high but performance is low, or vice-versa.
A normal RC-IV/CS ratio suggests balanced mitochondrial function and energy production. A low ratio may indicate impaired Complex IV activity, which can contribute to symptoms like fatigue, reduced exercise tolerance, neurological or developmental concerns, or other issues linked to mitochondrial dysfunction. A high ratio may reflect strong oxidative phosphorylation capacity or adaptive changes in mitochondrial efficiency.
Because buccal mitochondrial testing is a screening tool, this marker is best interpreted alongside other respiratory chain ratios (RC-I/CS, RC-II/CS, RC-II+III/CS) and clinical symptoms. Changes over time can provide additional insight into mitochondrial health and adaptation.
