What this test is

Glucose 1-hr is part of an Oral Glucose Tolerance Test (OGTT). After fasting, you drink a measured amount of glucose, and blood sugar is checked at timed intervals. The 1-hour value captures the early peak in blood glucose and can reveal problems earlier than fasting glucose alone.

Why it matters

Many people with early insulin resistance have normal fasting glucose but higher post-meal or post-load glucose spikes. A higher 1-hour glucose can indicate that your body needs more insulin (or has trouble using insulin effectively) to manage the sugar load. Over time, repeated glucose spikes may contribute to inflammation, triglyceride elevation, fatty liver risk, and progression toward prediabetes or type 2 diabetes.

What a high Glucose 1-hr can mean

Depending on the protocol used and your overall results, a high 1-hour glucose may be associated with:

  • Poor sleep, chronic stress, or shift work

  • Low physical activity, loss of muscle mass

  • Certain medications (e.g., steroids), acute illness/inflammation

  • Underlying endocrine issues (less common)

Possible symptoms when post-load glucose spikes are significant:

  • Energy crash 1–3 hours after meals

  • Increased thirst, frequent urination (more common with higher levels)

  • Brain fog, headaches, irritability

  • Increased hunger shortly after eating

What a low Glucose 1-hr can mean

A low 1-hour glucose is less common and may reflect:

  • A strong/rapid insulin response (glucose clears quickly)

  • Reactive hypoglycemia pattern when later values (2–3 hours) drop below normal

  • Prolonged fasting before the test or inadequate carbohydrate intake leading up to an OGTT (can affect response)

Symptoms (if glucose drops too quickly later): shakiness, sweating, anxiety, palpitations, weakness, or intense hunger.

What’s “normal” depends on the test protocol and the rest of your OGTT. A single 1-hour value is most helpful when interpreted alongside fasting glucose, 2-hour glucose, and insulin levels.

  • Review the full OGTT curve: fasting, 1-hour, 2-hour (and 3-hour if available)

  • If elevated: focus on reducing glucose spikes with:

    • Protein + fiber first at meals (vegetables, legumes, lean proteins)

Important interpretation notes

  • OGTT preparation matters: very low-carb dieting before the test can alter results.

  • Acute illness, poor sleep, and stress can transiently raise glucose.

  • Pregnancy uses different testing and cutoffs; 1-hour pregnancy screening is not the same as a standard 75 g OGTT.
