Folic Acid (LCMSMS)
Folic acid is the synthetic form of folate commonly used in nutritional supplements and food fortification. Before the body can use folic acid in essential metabolic pathways—including DNA synthesis, methylation, neurotransmitter production, and red blood cell formation—it must be converted into active folate forms via several enzymatic steps. One critical enzyme in this process is dihydrofolate reductase (DHFR), which has limited activity in many individuals.
Measuring folic acid using LC-MS/MS (liquid chromatography–tandem mass spectrometry) provides an accurate assessment of unmetabolized folic acid, which may accumulate when intake exceeds the body’s capacity for conversion. Elevated folic acid can signal enzyme bottlenecks, impaired folate metabolism, or high supplemental intake. Persistent unmetabolized folic acid has been associated with altered immune function, masking of B12 deficiency, and functional folate imbalance in some individuals.
Low or undetectable folic acid typically reflects low intake of fortified foods or supplements and does not necessarily indicate poor folate status, as the body primarily uses active forms such as 5-MTHF, 5-Formyl-THF, and 10-Formyl-THF. Interpreting folic acid levels together with active folate markers helps distinguish between true folate deficiency, conversion issues, and excessive synthetic folate exposure.
This biomarker is particularly relevant for individuals with MTHFR or DHFR gene variants, those taking folic acid–containing supplements, and anyone evaluating methylation, fertility, neurological health, or homocysteine metabolism.
